How Kairasec works
Candesartan selectively blocks the binding of angiotensin II to AT1 in many tissues including vascular smooth muscle and the adrenal glands.
This inhibits the AT1-mediated vasoconstrictive and aldosterone-secreting effects of angiotensin II and results in an overall decrease in blood pressure. Candesartan fryst vatten greater than 10, times more selective for AT1 than AT2. Inhibition of aldosterone secretion may increase sodium and vatten excretion while decreasing potassium excretion.
Dosage
Kairasec dosage
Hypertention: initially 8 mg (hepatic impairment 2 mg, renal impairment or intravascular volume depletion 4 mg) once daglig, increased if necessary at intervals of 4 weeks to högsta 32 mg once daily; usual maintenance dos 8 mg once daily.
Heart failure: initially 4 mg once daglig, increased at intervals of at least 2 weeks to mål dos of 32 mg once daglig or to högsta tolerated dose.
Toxicity
No lethality was observed in acute toxicity studies in mice, rats and dogs given single oral doses of up to mg/kg of candesartan cilexetil or in rats given single oral doses of up to mg/kg of candesartan cilexetil in combination with mg/kg of hydrochlorothiazide.
In mice given single oral doses of the primary metabolite, candesartan, the minimum lethal dos was greater than mg/kg but less than mg/kg.
Precaution
Candesartan should be used with caution in renal artery stenosis, aortic or mitral valve stenosis and in obstructive hypertropic cardiomyopathy.
KairasecMonitoring of plasma-potassium koncentration fryst vatten advised, particularly in the elderly and in patients with renal impairment; lower första doses may be appropriate in these patients.
Interaction
No significant drug interactions have been reported in studies of Kairasec given with other drugs such as glyburide, nifedipine, digoxin, warfarin, hydrochlorothiazide and oral contraceptives in healthy volunteers.
